Substance Use Assessment & Evaluation

New Opportunities, Inc. offers a comprehensive and confidential assessment which includes counselor/client interview, use of evidence based assessment questionnaire, collateral information and drug/alcohol testing.  We will assess the individual’s use of substances to determine if there is a substance use disorder, if treatment services are needed and what level of treatment is most appropriate.  The assessment will be completed by a credentialed clinician and can take approximately 1.5 -2 hours to complete.  The evaluation process is completed in a confidential manner and all records are protected by State, Federal and HIPAA laws governing confidentiality and privacy.

New Opportunities, Inc. accepts most private insurances and Medicaid.  We also have a sliding fee scale for assessments and treatment sessions that are based on family size and income. 

To qualify for the sliding fee scale, this documentation must be brought to the first appointment:

  • Proof of Iowa residency: proof of residency would include but not limited to driver’s license, state of Iowa photo identification.

  • Proof of income: proof of income may include, but is not limited to, a recent pay check stub, a bank statement, the past year’s income tax return or a report from Iowa Workforce Development. 

If you would like to have a substance use disorder assessment please contact our Carroll office at 712-792-9266.  All appointments for New Opportunities, Inc. offices in Audubon, Sac City, Jefferson and Guthrie Center are scheduled by our staff in the Carroll location.  You will be able to attend the actual assessment appointment in the outer office.
