Walk-In appointments for assessment/evaluation
Available in the Carroll office only.
Evaluations are available on a first come/first served basis on the following days/times:
Carroll office Walk-in day and time: Thursday 8:30am -12:30pm
What to expect: Arrive at the office, check in, first come/first served basis, depending on wait time please plan on minimum of 2 hours.
Things to bring:
- Insurance card
- Drivers License/Photo ID
- Proof of Income (i.e. pay stub, taxes)
- Medication Information
- Legal information (i.e. court case number)
